Parental Authorization Form - NFAA State/Sectional/National Events

Parental authorization is required for either of the following situations (please check which applies):

• Age Class Move-up: An archer wishing to compete in a higher age class than his/her birthdate would normally qualify for. NFAA Constitution, ARTICLE III, Section B, Sub-section 1.2
• Professional Membership: An underage archer/minor wishing to compete in the Pro division.
